Job Overview – Registered Nurse (Clinical Care Manager):
Compensation: $57 – $61/hour
Location: Queens, NY
Schedule: Monday to Friday, 8:30 AM to 5:00 PM
Join our client’s team as a Registered Nurse (Clinical Care Manager) in Queens, NY, leading care coordination and clinical quality in a community-based setting. This role involves managing IDT meetings, overseeing care transitions, mentoring staff, and ensuring regulatory compliance. Candidates should have experience in long-term care, home health, or managed care, with strong EMR and UAS assessment skills.
Responsibilities as the Registered Nurse (Clinical Care Manager):
- Care Coordination & Quality Oversight: Manage individualized care plans, facilitate care transitions, and ensure high-quality outcomes across clinical settings.
- Team Leadership & Staffing: Supervise nursing staff, oversee scheduling and performance, and support training, development, and HR processes.
- IDT Collaboration & Service Delivery: Lead interdisciplinary team meetings and support service delivery requests to align care with participant needs.
- Regulatory Compliance & Documentation: Ensure complete, accurate records across all participant touchpoints while maintaining compliance with all regulations.
- Clinical Review & Incident Management: Monitor clinical documentation, address chart deficiencies, and lead investigations to improve safety and care standards.
Qualifications for the Registered Nurse (Clinical Care Manager):
- Education: RN from an NLN-accredited program required; Bachelor’s degree in Nursing (BSN) preferred.
- Licensure: Active and unrestricted Registered Nurse license in New York State required.
- Certification: Completion of NYSED-approved infection control coursework required.
- Experience: 2+ years in a clinical management role (CHHA, LHCSA, acute or long-term care), with a minimum of 1 year working with elderly or frail populations.
- Technical Skills: Skilled in EMR systems with experience in UAS assessments and data analysis for quality improvement.
- Skills & Attributes: Detail-oriented and organized leader with strong clinical judgment, communication skills, and a proactive, team-based approach.
- Language Requirement: Bilingual in Korean and English strongly preferred.
