Job Description

The Assistant Director of Nursing (ADN) - Service Delivery will be responsible for providing oversight to a clinical team in the coordination and rendering of care delivery in the Certified Home Health Agency. This team will include Registered Nurse but may extend to other members of the interdisciplinary care team. The Assistant Director of Nursing- Service Delivery is a part of the leadership team that will focus on ensuring the assigned clinicians strive to achieve the quality patient care outcomes that are steeped in evidence-based care and practices. In addition, the

Assistant Director of Nursing- Service Delivery will engage in efficient advisement, coaching and mentoring strategies to the members of the care team in order to meet clinical and professional goals. The Assistant Director of Nursing- Service Delivery will stay abreast of industry trends and regulatory guidelines that govern the home health care team and clinical practice.

Minimum Qualifications

  • Licensed and currently registered to practice as a professional nurse in New York State, plus (2) or (3), below.
  • Master’s degree in nursing administration, Education, Clinical Specialty or equivalent field and five years of satisfactory nursing experience, of which three years shall have been in a supervisory or teaching capacity; or a Baccalaureate Degree in Nursing and six years of satisfactory nursing experience, four years of which shall have been in a supervisory or teaching capacity.
  • A satisfactory equivalent of education and experience. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.

Department Preferences

Years Of Experience

Minimum two (2) years of experience in a Certified Home Health Agency

Minimum of 2 years of Public/Community Health and 2 years medical/surgical experience required. MRDD experience preferred.

Previous experience in PI, i.e., knowledge of PI applications, tools and techniques and knowledge of the development, coordination and facilitation of PI programs.

Previous experience using various computer software applications: i.e., Word, Excel, Access, etc

AND- meet at least Four (4) of the following criteria:

 At least One (1) to two (2) years home health care experience in a Certified Home Health Agency in a leadership role

 Demonstrated knowledge of the Home Health Conditions of Participation, DOH/CMS and Joint Commission Regulations.

 One (1) to two (2) years acute care experience

 One (1) to two (2) years acute care experience

AND

 One (1) to two (2) years' experience as a CHHA nurse, CHHA Case Manager or CHHA Coordinator of Care AND

 One (1) to two (2) years demonstrated experience in the role of Health Care Administrator in a home health care agency.

Knowledgeable In

A satisfactory equivalent of education and experience.

However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.

Demonstrated knowledge regarding Federal, State and local regulations and JCAHO standards for Home Care.

Excellent verbal, written, organizational and analytical skills.
