RN - Case Management New

Job Title: RN - Case Management

Location: Bronx, NY 10461

Schedule: 9:00 AM-5:00 PM, Mon-Fri

Duration: 8 Weeks with possible extension

Local Pay Range: $65 - $72/Hour.

Required Experience:

• Two years of case/resource management and utilization management are required.

Duties:

• Coordinate and manage patient care plans to ensure timely and appropriate services.
• Collaborate with interdisciplinary teams to optimize patient outcomes and resource use.
• Perform utilization reviews and ensure compliance with insurance and regulatory requirements.
• Provide patient education and facilitate discharge planning.
• Monitor and evaluate care plans, adjusting based on patient progress and needs.

Education:

· BSN

Required Certificates:

• New York State RN License
• AHA BLS
