Position Description

A social services agency in New York City is looking to add a new Licensed Practical Nurse to their growing Foster Care Program in Brooklyn. Working under the Nurse Manager, the LPN will be responsible for overseeing medical care management of the youth in family foster care.

About the Opportunity:

  • Start Date: ASAP
  • Assignment Length: Contract to Perm
  • Schedule: Monday to Friday
  • Hours: 9am to 5pm or 10am to 6pm (35 hours per week
  • Setting: Foster Care

Responsibilities:

  • Provide direct nursing care to residents, including administering medications, monitoring vital signs, and performing routine health assessments.
  • Collaborate with healthcare providers, social workers, and other staff to develop and implement individualized care plans.
  • Educate residents on health and wellness topics, promoting healthy lifestyle choices.
  • Maintain accurate and up-to-date medical records for all residents.
  • Respond to medical emergencies and provide first aid as needed.
  • Participate in staff meetings, training sessions, and continuing education opportunities.
  • Ensure compliance with all healthcare regulations and standards.
  • Provide specialized care and support for teenage boys and girls, addressing their unique physical and emotional needs.
  • Assist in managing behavioral health issues, including providing support for residents dealing with trauma, anxiety, and other mental health challenges.
  • Facilitate health education sessions tailored to the needs of adolescents, covering topics such as sexual health, substance abuse prevention, and nutrition.
  • Work with residents to develop healthy coping mechanisms and life skills, promoting overall well-being and resilience.
  • Coordinate with family members and guardians to ensure continuity of care and support for residents.

Qualifications:

  • Graduate of an accredited Practical Nursing program
  • NYS Licensed Practical Nurse (LPN)
  • CPR certification
  • Bilingual (English and Spanish Preferred)
